Northern Light Acadia Hospital Department: Integrated Behav Hlth ClinicPosition is located: Northern Light Cutler Health CenterWork Type: Part Time EligibleHours Per Week: 20.00Work Schedule: 7:00 AM to 5:00 PMSummary:The Integrated Behavioral Health clinician III is an independently licensed LCSW or LCPC embedded within an outpatient medical setting as a psychotherapist. They directly with the primary care team to assess and treat individuals with behavioral health and/or substance use issues (either on-site or via telemedicine) . They understand or are willing to learn about the collaborative model of care applied in a patient-centered medical home. They adapt interventions to the workflow and pace of the primary care practice, including rapid assessment, brief treatment, and a high daily volume of patient contacts. They frequently consult with primary care providers in real time to assist patients with psychosocial issues and utilize "warm hand offs" during hours of clinical operation to enhance patient engagement with the primary care team. They are a clinical leader, a consultant, and a resource at the medical practice in program development, actively participating in group decision-making, implementing plans, and improving quality of care. They are an enthusiastic learner who actively seeks research and knowledge to ensure current, evidence-based practice while adhering to key components of collaborative care. This clinician performs duties and/or responsibilities within their scope of practice as assigned by the Clinical Supervisor of Behavioral Health Integration. To be successful in this role: •The LCSW deeply enjoys being a therapist and has a desire to work directly with patients •The LCSW is highly self-motivated and comfortable working autonomously as well as collaborativelyResponsibilities:Functions as an informal leader within program area and larger organization by providing ongoing feedback to colleagues on evidence-based practices and outcome informed care.Provides clinical supervision to clinicians as assignedDevelops and presents educational programs aligned with professional interests and skills.Maintains an active presence (either in person or virtually) on primary care clinical teams. Participates in medical team huddles, medical staff meetings, multidisciplinary teams, and individual/group supervision sessions.Works with patients to provide treatment interventions for behavioral health/SUD concerns and develops whole health and wellness recovery plans.Matches and adjusts the type and intensity of services to the needs of the patient, ensuring the timely and unduplicated provision of care.Consults with primary care providers in real time to assist patients with psychosocial issues.Ensures the flow and exchange of information among the patient, family members, and linked providers.Provides or arrange access to "patient navigation" services that focus on benefits and financial counseling, transportation, home care, and access to social services, peer support, and treatment.Documents in the patients' electronic health record in a complete and timely manner. Completes and signs all progress notes within 24 hours of the encounter.Triages both scheduled and immediate referrals from the medical teams, treating some with brief treatment/intervention and referring others to more appropriate resources.Advises the primary care provider about which patients are better served at the primary care setting and which should be referred for specialty services.Performs supervisory functions in the absence of clinical supervisor for frontline clinicians as needed. Maintains and abides by ethical standards set forth by the facility and Maine State Licensing Boards.Maintains requirements of licensure.Fosters the development of teamwork within department as well as across departments and with outside providers.Gives and receives critical feedback on effectiveness of clinical interventions through regular peer review, scheduled staff meetings and supervisions.Utilizes time effectively and can critically evaluate time management relevant to the effective operation of respective department.Other Information:Competencies and SkillsBehaves with Integrity and Builds Trust: Acts consistently in line with the core values, commitments and rules of conduct.
