The Adult Day Health Care (ADHC) Program at client is a unique community-based health care program that provides comprehensive nursing, social work, nutrition, mental health and substance use services to individuals living with chronic illnesses including HIV, behavioral health conditions, diabetes, hypertension and others is seeking a FNP or ANP to serve as lead nurse, responsible for oversight of the nursing services at our locations, Manhattan and Downtown Brooklyn
Type: Full-Time/ Direct-Hire
Shifts: Mon-Fri, **no weekends
Location: New York
Summary - The Family or Adult nurse practitioner is responsible for the provision of medical care in a community health center, which is recognized as a Level 3 Patient Centered Medical Home, and which is a Federally Qualified Health Center, with a homeless program.
- The Family or Adult nurse practitioner will report to the Medical Director, or their physician designee.
- He/she will provide collaboration and supervision to the Nurse care coordinator.
Essential Functions - Performs complete history and physical examinations in accordance with current standards of professional practice, regulatory requirements and agency policies and procedures.
- Provides primary medical care to adults and manages acute and chronic conditions.
- Provides MAT and low threshold Buprenorphine therapy.
- Assesses patients' conditions and age-specific medical needs; sets goals and prescribes appropriate clinical interventions to meet those goals.
- Provides complete and appropriate documentation in the electronic health record regarding clinical observations, diagnoses, treatment objectives and patients' responses to treatment regimens
- Maintains patient confidentiality.
- Collaborates effectively with other health care providers in planning interventions to best meet patients' needs in a timely manner and to promote the continuum of care.
- Prescribes appropriate laboratory and diagnostic procedures related to patients' health status and facilitates appropriate treatment interventions predicated on results of these procedures
- Performs direct patient care procedures including but not limited to venipuncture and in accordance with agency policies and procedures, acceptable standards of practice, and in accordance with approved delineation of privileges
- Participates in the development of standards of care as assigned by the Chief Medical Officer and/or Chief Operating Officer.
- Orders medical subspecialty consultation and evaluates for the need for hospitalizations and coordinates the transfer of the resident/patient.
- Provides emergency medical care as maybe required by resident/patient, staff and visitors.
- Provides patient advocacy.
- Participates in continuing education programs appropriate to the practice.
- Participates in the Quality Maintenance Program at the request of the committee.
- Participates in the meetings of the Medical Department.
- Maintains confidential case records, medical charts and files pertaining to the resident/patient.
- Provides health promotion/disease prevention education specific to the needs of MOUD patients.
- Participates in the development of patient care protocols.
- Maintains the Quality Assurance Program that defines methods for the identification and selection of clinical and administrative problems, along with strategies for corrective action to better provide services in accordance with current standards of professional practice
Minimum Requirements - Has current New York State licensure and registered as a Nurse practitioner.
- Experience in Primary Care and clinical management of persons with HIV/AIDS
- Experience in harm reduction and Buprenorphine treatment
- Certified in field of Nursing practice, Family and/or Adult Medicine and/or demonstrated expertise and experience in HIV/AIDS medicine
- Previous experience with an electronic health record, preferred
- Visual acuity and manual dexterity to interface with the computer and perform medical procedures as indicated
- Auditory acuity to handle phone calls and interact extensively with patients.
- Knowledge of HIPAA compliance
- Maintains current DEA registration
- Maintains current CPR certification
- Maintains CMEs/CEUs required for licensure
- Maintains current Infection Control certification
- Bilingual English/Spanish preferred, but not required

Compensation Range: $150,000 commensurate with experience
